Position Summary/Description 
HSO is developing a new edition of the reproductive health standards. The new standard will provide guidance to enable the delivery of safe, equitable, people-centred, comprehensive, coordinated, and continuous reproductive health care services. It will support meeting the needs and preferences of individuals, families, caregivers, and communities served, and apply to all populations, all ages, and all health care settings where reproductive health services are delivered. These settings include hospitals, clinics, primary care, community health services, and specialized reproductive health centers.
The standard will address social determinants of health, ethical considerations, and cultural safety in reproductive health care. It will cover a range of services including obstetrics, perinatal care, assistive reproductive technologies, and reproductive health services such as contraception, fertility preservation, and gynecological care. This includes integrating health and social services, providing system navigation support, ensuring equitable access to reproductive care, and addressing barriers faced by underserved populations.
The standard will outline roles, responsibilities, planning, performance, safety, risk management, equity-informed care, and quality improvement to enhance reproductive health care delivery. It will include measurable, outcomes-based requirements to improve the quality, safety, and accessibility of reproductive health services, ensuring they are delivered in alignment with evidence-informed practices, ethical principles, and legal requirements.

In your role as a TC member, you will:
  • Work with key interest holders and experts from Canada and internationally in the revision or development of HSO standards under the Technical Committee’s purview.
  • Bring the perspective of your professional or lived experience (e.g. patient, health provider, policymaker, researcher, health administrator, etc...).  
  • Advise and reach consensus on the topics for inclusion and the content of the standards through a series of meetings.
  • Maintain effective communication and knowledge transfer amongst Technical Committee members and the HSO staff.
  • Provide guidance to HSO staff on standard(s) related interpretation questions and inquiries related to technical content of the standard(s).
  • Promote the adoption of standards once published.

The acute care standards will provide guidance to enable the delivery of safe, equitable, people-centred, comprehensive, coordinated, and continuous services across various acute care domains. These standards will address the needs and preferences of patients, families, caregivers, and communities and will apply to all populations, age groups, and acute care settings. Collectively, these standards will focus on emergency medical transport, emergency medical services, stroke, critical care, trauma care, and inpatient services.
Each standard will address the social determinants of health, equity in access to acute care, and the unique challenges faced in diverse contexts, including rural, remote, and urban settings. Individually, these standards will cover specific domains of acute care, including the provision of timely and effective emergency medical transport, life-saving interventions in emergency and critical care settings, trauma stabilization, and patient-centred inpatient care. Each standard will also emphasize effective transitions of care, system navigation, and integration with other health services to ensure continuity and coordination.
The acute care standards will collectively outline roles, responsibilities, planning, performance, safety, risk management, equity-informed care, and quality improvement initiatives to enhance acute care delivery. Each standard will include measurable, outcomes-based requirements tailored to the respective domain, ensuring the delivery of high-quality, safe, and efficient acute care services in alignment with evidence-informed practices, principles, and regulatory frameworks.

Social Worker

Le Foyer Saint-Thomas de la Vallée de Memramcook is a non-profit organization that operates long-term care homes. In the Memramcook region, we operate the Dr. Camille-E-Gaudet home, a 30-bed level 3 home and Le Domaine La Vallée, a 30-bed level 2 adult residential facility. We also operate the Foyer Notre-Dame-du-Sacré-Coeur, a 20-bed level 3 home in Dieppe. Our organization also offers 77 housing units for independent seniors in the Memramcook region. Our organization's mission is to provide optimal quality care and services for seniors and/or people with special needs.


As part of the provincial initiative of the "Nursing Home Without Walls" program (FR), we want to offer seniors in Memramcook and Dieppe the opportunity to age in place, in the comfort of their own home for as long as possible. If you are someone who is passionate about teamwork and working in a family environment of collaboration and mutual aid, Le Foyer Saint-Thomas is the place for you! We are currently looking for a dynamic person to fill the position of Social Worker within our "Nursing Home Without Walls" program team.


TYPE OF POSITION: Temporary full-time for a period of 1 year with the possibility of renewal.
WORK LOCATION: Municipalities of Memramcook and Dieppe
WORK SCHEDULE: 75 hours per 2 weeks. Monday to Friday. Work schedule may change depending on operational needs.

 

FUNCTION: Working closely with and reporting to the Executive Director, the Social Worker participates in the development and implementation of the guidelines for the Memramcook and Dieppe region’s “Nursing Home Without Walls” programs. The objectives are:

Support the development and delivery of a community outreach program for the senior population (60+) to achieve the project objectives.
• Ensure that seniors and their families/friends/neighbours have access to appropriate services and information related to aging in place;
• Propose social health initiatives to address social isolation and loneliness among seniors and their caregivers;
• Improve knowledge of health issues important to aging in place and healthy aging among seniors and their caregivers;
• Empower the local community to meet the needs of an aging population

Support the assessment of how social and built environments can support healthy aging by developing and implementing assessment tools and reporting on initiatives. Initiatives include:
• Building on the premises of the Home First Strategy and the New Brunswick Aging Strategy and Family Plan, providing the services and support options needed to age in place.
• Developing programs to address social isolation.
• Providing health promotion opportunities.
• Providing a supportive community environment for seniors and their caregivers.
• Disseminating information that supports aging in place.

Examine the capacity of community-based approaches to address health inequities.
• Support interventions for aging in place.
• Build capacity for social contacts so that vulnerable older adults can remain safely in their homes.
• Provide opportunities for community members to collaborate with the retirement home to support older adults in their community.
• Help identify best practices for aging in place, building on existing infrastructure that is available in most communities.
• Advocate for evidence-based best practices.
• Assess the needs of older adults.

MINIMUM REQUIREMENTS:
• Bachelor of Social Work;
• Active member of the New Brunswick Association of Social Workers;
• Good knowledge of aging, senior-friendly communities and resources, and issues related to the senior population.
• Thorough understanding of performance measurement and evaluation.
• Thorough understanding of project management.
• Excellent self-motivation and problem-solving skills.
• Excellent communication skills, both written and oral, and the ability to build positive relationships and collaborations.
• Ability to present information to individuals and groups.
• Experience engaging people through assessments and/or group work is an asset.
• Knowledge and experience in program development and implementation;
• Good computer skills and Microsoft Office skills;
• Good previous work record (performance and attendance);
• Physical ability to perform assigned work;
• Adherence to professional ethics and organizational values;
• Respect for confidentiality rules;
• Knowledge of spoken and written French is essential and must provide service in both official languages.
