Lodge a Complaint

REQUIREMENTS FOR APPLICATION AS A TEMPORARY MEMBER

 

Please note The documents presented must be originals (except for the proof of identity).

 

1.  DOCUMENT # 1 : APPLICATION FOR MEMBERSHIP FORM

Please complete the form, sign it, and return it to the Association office accompanied by the $60.00 non refundable registration fee.

 

2.  DOCUMENT # 4 : CERTIFICATION OF COMPLETION OF NECESSARY REQUIREMENTS FOR SOCIAL WORK DEGREE

Please complete the top section, sign, and forward to the university, from which you will be receiving your degree in order that they may complete the bottom section, affix their seal, and return the completed form directly to the Association office.  If the university is unable to affix the seal to the affidavit, please have the university return the completed affidavit (without the seal) accompanied by an official transcript of marks.

 

3.  DOCUMENT # 3 : REFERENCES

Please refer to the “policy regarding the references” (Appendix A)

Please ask the persons that will provide these references to forward them directly to the Association.

 

4. DOCUMENT # 5 : TEMPORARY MEMBERSHIP SUPERVISORY AGREEMENT

This form must be signed by a registered social worker who will supervise you until such time as you have your degree in your hand and you can become a regular member of the NBASW

 

5. DOCUMENT #6: AGREEMENT REGARDING AFFIDAVIT OF DIPLOMA

COMPLETE AND RETURN WITH APPLICATION

 

6. CRIMINAL RECORD CHECK

Please refer to the "Policy Regarding the Criminal Record Check" (Appendix B)

 

7.  PROOF OF IDENTITY

You are asked to provide copies of two pieces of identification that show your present and/or former legal names, date of birth and date of change of name.  Examples of such documents are: birth or baptismal certificate, marriage certificate, legal change of name document, passport or immigration document, driver’s licence, Medicare and hospitalization cards.

 

8.  DOCUMENT #2 – AFFIDAVIT OF DIPLOMA

After convocation, please complete the top section, sign, and forward to the university, from which you will receive your degree in order that they may complete the bottom section, affix their seal, and return the completed form directly to the Association office.  If the university is unable to affix the seal to the affidavit, please have the university return the completed affidavit (without the seal) accompanied by an official transcript of marks.

 

9.  REGISTRATION FORM (LONG FORM)

Unless this is an urgent matter, this form will only be sent to you with the letter informing you that you have been approved for membership.  However, to accelerate the process, you may request this form, and it can be sent with the appropriate dues with the original documents. The Association will hold them until approval.  Otherwise, this form is to be completed, dated, signed and returned to the NBASW office with appropriate dues after receiving notification of   approval.

 

In addition, where there is any other evidence which indicates a person may be unsuitable for registration in the Association; the person’s application may be subject to further review by the Board in conjunction with the Committee of Examiners and/or the Registrar.
