Please ask the persons that will provide these references to forward their letters directly to the Association.

If you have worked in another province, the Registrar will contact that province to verify if you were registered there and whether you were the object of any complaint and disciplinary action while in that province.
7. CRIMINAL RECORD CHECK

8. PROOF OF IDENTITY
You are asked to provide copies of two pieces of identification that show your present and/or former legal names, date of birth and date of change of name. 9. REGISTRATION FORM (LONG SHEET)
Unless this is an urgent matter, this form will only be sent to you with the letter informing you that you have been approved for reinstatement. If not approved, the dues will be returned. Otherwise, this form is to be completed, dated, signed and returned to the NBASW office with appropriate dues after receiving notification of approval.
In addition, where there is any other evidence which indicates a person may be unsuitable for reinstatement in the Association, the person’s application may be subject to further review by the Board in conjunction with the Committee of Examiners and/or Registrar.
